Register As A New Patient
We are delighted you are joining Bacon Lane Surgery as your GP Surgery. To register, simply fill the below form for each patient who wishes to register e.g. if you are registering a family of four, you will need to fill the form 4 times (once for each person).
The online form is the quickest way to register but if you are having any difficulties filling the form, please collect paper forms from reception. You will need one for each person you wish to register. Once you have filled the paper form, kindly return this to reception.
