New Patient Registration Document Requirements
As a practice, we are obliged to request documents from you as part of your registration. These are one proof of Identity (ID) and one proof of address. Once we have done the needful, they will be destroyed.
If you do not have any documents e.g. you are homeless or for other reasons, please let us know when registering. This will not stop you registering but we may have to ask you to attend the surgery to confirm identity.
Proof of Identity (ID) can be:
- Passport (Recommended)
- Driving License
- Other form of photographic ID
Proof of Address can be:
- Bills Eg Council Tax, Water, Energy
- Statements – Bank or Credit Card
- NB – Your name and address need to both be visible.
You can provide these documents by either:
- Upload this on the form when you register online (Recommended)
- you can email it to baconlane.surgery@nhs.net
- You can also bring in the physical documents to the surgery where we can take a copy (NB we do not keep the originals)

1300-1400 - Phone lines may be closed for practice meeting. We will reopen them as soon as the meeting is completed - Saturday
