Order your Repeat Prescriptions

Please use the form below to request a repeat prescription. Once submitted, this will take 3 working days as per our practice policy.

If you require an urgent prescription (within 24 hours), please request the medication in writing and call the surgery on 02089525073. We will try and accomodate your request.

We must have a written request and cannot accept requests over the phone. There are 4 ways to request a prescription:

• Fill the right hand side of your repeat prescription and drop this off at the surgery
• Use the NHS App or Patient Access App to request your medications (Recommended)
• Email baconlane.surgery@nhs.net to request your medication
• Ask your pharmacy to request medication on your behalf (only if you cant do any of the above)
