  • Please list one medication per box then click the + button on the right hand side to another medication. Use one line per medication being requested
  • Please tell us which pharmacy to send your prescription to. You do not need to collect your prescription. Simply collect your medication directly from the pharmacy. Please include the pharmacy name and address. Once setup, you do not need to do this again and can change the pharmacy anytime, just let us know. We can even send a prescription as a one off to another pharmacy.
